TIA is offering its members HM Care Advantage, a group limited benefit medical indemnity insurance plan. TIA’s exclusive offering gives you three plans to choose from that include unlimited physician visits, insured prescription drug, a daily in-hospital benefit, an additional first day of admission benefit, surgery and anesthesia, outpatient diagnostic testing, and more!
Employees can choose coverage for themselves or coverage for their spouse and/or children. In addition, a national provider network is included to further reduce an employee's out-of-pocket expenses. And, member identification cards show each covered benefit.
For ease of administration for the employer and their employees, payroll deduction is offered, along with a customer service center for claims and questions. This limited benefit medical plan is available through First National Administrators (FNA) and Trade Insurance Brokerage (TIB), a respected insurance and benefits administration firm, and underwritten by HM Life Insurance Company.

Networks Help Reduce Members’ Expenses
A national provider network is offered to reduce the out-of-pocket costs to members when a network physician, hospital or facility is used. There is no penalty if a non-network physician, hospital or facility is used. Discounts continue even if specified benefits are exhausted.
Age GuidelinesEmployees and spouses ages 18 to 69 years may enroll for coverage. Benefits terminate at age 70.
Active Work Requirement
An eligible employee must be actively at work, working at least an average of 15 hours per week for the group policyholder (to be verified annually using the quarterly wage report).
Employer-Sponsored DiscountsEmployer-sponsored plans may be discounted up to 25 percent, depending on the amount of the contribution provided. The discount does not apply to insured dental, vision or prescription drug coverage or the value-added services.
